The Commonwealth’s only VA health facility, in Garapan on Saipan, open two days a week for primary care. Everything beyond that is telehealth, a travelling provider, or a flight to Honolulu.
Also called: Saipan VA Clinic, VA clinic, Garapan, Marina Heights, VAPIHCS, Pacific Islands Health Care System, telehealth, Honolulu, Tripler.
The Commonwealth has one VA health facility and it is open two days a week.
The Saipan VA Clinic is at Marina Heights Business Park in Garapan, in the Medical Associates of the Pacific MH-II Building, Suites 100 and 206. VA publishes its hours as Tuesday and Thursday, 8:30 a.m. to 4:00 p.m., closed the rest of the week. It offers primary care.
In VA's own accounting the Commonwealth does not have a community-based outpatient clinic at all. The Government Accountability Office's 2024 review of VA in the territories counted, for the Northern Mariana Islands, zero VA medical centers, zero community-based outpatient clinics, one other outpatient clinic and one Vet Center or outstation. Guam and American Samoa each have a full community-based outpatient clinic; the Commonwealth has the smaller category.
Everything above primary care is somewhere else. GAO found that specialty care at the outpatient clinics in American Samoa, Guam and the Northern Mariana Islands is limited, and that specialty care from VA providers is generally delivered by telehealth, by providers travelling from other VA Pacific Islands facilities, or by referral to the Honolulu VA medical center. The Honolulu medical center has no inpatient hospital of its own; VA Pacific Islands meets that gap through sharing agreements with the Defense Department, chiefly Tripler Army Medical Center in Honolulu and U.S. Naval Hospital Guam, which is next door to the Guam clinic and is the nearer of the two for a Saipan veteran.
Community care is coordinated from Honolulu, through VA's Community Care Network Region 4, for veterans living in the Commonwealth as well as Guam and American Samoa.
The distances are the substance of this entry. GAO recorded VA's estimate that a round-trip flight between the Northern Mariana Islands and Hawaii cost more than $3,700 in January 2023, the highest of the figures it published. Whether VA pays for that trip is the beneficiary travel question, and it has its own entry here; GAO reported that fifty-six percent of the Commonwealth's VA-enrolled veterans were not eligible for travel benefits in fiscal 2022.
Enrollment is ordinary federal VA health care enrollment. There is no Commonwealth application and no local eligibility rule. VA Pacific Islands reported roughly 7,200 enrolled veterans across its three territorial clinics in fiscal 2023.
Practical advice that follows from the two-day schedule: ask for telehealth by default, get prescriptions on the mail-order pharmacy rather than in person, and when a referral is made, settle the travel question before you accept the appointment.

Eligibility and priority group follow the usual federal rules. There is no Commonwealth-specific application, and a service-connected rating changes priority and copays rather than opening or closing the door.
Two days a week, primary care only
VA publishes the Saipan clinic as open Tuesday and Thursday, 8:30 a.m. to 4:00 p.m. GAO counted the Commonwealth as having no community-based outpatient clinic, one other outpatient clinic, and one Vet Center or outstation.
Specialty care means telehealth, a travelling provider, Guam or Honolulu
GAO reported that specialty care at the Pacific territory clinics is limited and generally delivered by telehealth, by providers travelling from other VA Pacific Islands facilities, or by referral to Honolulu, which has no inpatient hospital and relies on Tripler Army Medical Center and U.S. Naval Hospital Guam.
