Where it applies

Written for the jurisdictions listed here.

Veteran Directed Care is a VA programme: VA gives an eligible veteran a monthly budget for home and community services, and the veteran, with an area agency on aging's help, chooses and hires the workers, often a family member or friend. In Tooele County the Health Department's Aging Services runs it: "A program for nursing home eligible veterans who want to live in their home and community as long as possible and who want to be cared for by someone they know and trust." The agreement. The County Council approved a memorandum of understanding with the Veterans Health Administration on October 5, 2021 (contract 21-10-05), under which "VA agrees to purchase VDC from an Area Agency on Aging" and the county is reimbursed for the services and its staff time; Aging Services described it as "an ongoing program that we will run from now on". On October 21, 2025 the Council raised the Aging Fund by $125,000 (Resolution 2025-25) because "Our veterans program is growing from 3-9 veterans served", and the 2026 budget sets the VA revenue at $325,000. How to get it. VA decides eligibility: ask your VA primary care team or social worker for a Veteran Directed Care referral, and tell them Tooele County Aging Services provides it.
