Where it applies

Written for the jurisdictions listed here.

Santa Rosa County Veterans Services, a Board of County Commissioners office, files VA claims and other VA benefits and answers questions about Florida's benefits for veterans; see the Florida entry on claims assistance. Its benefit pages cover new disability claims, increases, appeals, the Benefits Delivery at Discharge programme for members still serving, veterans pension with and without Aid and Attendance, DIC, survivors pension with and without Aid and Attendance, and burial benefits. How it works. Claims go to VA online, so VA writes only to the veteran or claimant; bring the rating decision back so the office can update its file. If an intent to file is already in place, through VA.gov, VA or the office, the claim and evidence are due a year to the day after it; the office asks to hear from the veteran at least 60 days before that deadline. Medical records must be on a USB drive or disc. For a burial claim it asks for every DD-214, the marriage certificate and the history of earlier marriages, the long-form death certificate, the funeral home's statement showing the balance paid in full, transport costs, a government ID and a voided check. Offices, lobby hours Monday to Friday 9 a.m. to 3 p.m.: Milton, 6051 Old Bagdad Highway, Suite 204; Gulf Breeze, 5841 Gulf Breeze Parkway; Jay, 3927 Highway 4, by appointment only. Phone 850-981-7155; email [email protected].
