The form demands "A signed Louisiana Department of Veteran's Affairs Form A25 (Revised 12.22)" with it, and closes "I UNDERSTAND THAT IT IS A CRIMINAL OFFENSE TO MAKE ANY FALSE STATEMENTS FOR THE PURPOSE OF PROCURING THIS EXEMPTION."

The surviving-spouse affidavit form (2026) sits on the same page, with the unmarried surviving spouse's full homestead under La. Const. 7:21(M) and the $102,700 income freeze.
